Once we schedule our initial consultation, you will receive an email to the PracticeQ portal with links to complete a confidential intake packet. This must be completed 48 hours prior to our first appointment.
I am in network with most United/Optum plans. I am an "out of network provider" for all other insurances. EquiMind LLC does not participate in Medicare or Medicaid and you cannot submit any bills from EquiMind to Medicare/Medicaid. ​All patients must have an active credit card on file in order to receive treatment at EquiMind and payment is due in full at the time of service. Most FSA/HSA spending plans are also accepted as payment.
If I am out-of-network with your insurance, I can provide you with documentation including what you paid along with medical billing codes that reflect services provided (called a "superbill") which you can submit to your insurance for partial reimbursement if you have out-of-network coverage. I am not able to negotiate with insurance companies. Please consult your insurance provider in advance for information regarding behavioral health coverage.  ​
Appointment Fees (eligible for coverage/reimbursement from insurance): ​Initial Consultation (60 minutes): $350​ 30-minute follow-up (Med Management Only): $175 ​ 50-minute follow-up: $250​​ Other Fees (Billed to credit card on file and NOT eligible for coverage/reimbursement from insurance):
Preparation of Medical Records: $25 Same Day Refill Request: $35 ​Phone calls, forms, reports, and other paperwork that require more than 5 minutes to complete outside of scheduled appointments will be billed at $350/hour.

Your scheduled appointment time is reserved time in which you have my full attention. If you arrive late, we will finish at the scheduled time. Extending your appointment time would result in delays for other patients.
Once an appointment is scheduled, you are responsible for the full professional fee unless you provide advance notice. Notice of cancellation must be received at least 24 business hours in advance of your appointment.  Business hours are considered weekdays from Monday through Friday and exclude all standard holidays. You will receive auto-generated reminders about upcoming appointments. Late cancellation for our initial consultation may not be rescheduled, based on a number of factors. If you do not come in for our initial meeting without contacting me beforehand, you will not be able to reschedule and will need to find another mental health provider.
Cancellations made prior to one business day before the appointment are not charged. If a cancellation occurs less than one business day before the appointment, you may incur a charge equal to half of the visit cost. For missed appointments, you will be charged the full cost of the visit. These fees cannot be submitted for insurance reimbursement. Repeated missed appointments or late cancellations may be cause to discontinue treatment. Given potential unforeseen circumstances, the first late cancellation or missed appointment​ will be forgiven for established patients.​
In the event that I need to reschedule an appointment, I will make every effort to provide you with adequate notice and will work to reschedule your appointment in a timely fashion.​​​
Due to concerns of medication accuracy and concerns for your privacy, I do not respond to pharmacy-generated automated refill requests.  Before requesting medication refills, please contact the pharmacy directly to ensure that you do not have any additional refills or a new prescription available. Please contact me for medication refills 5 days prior to needing the medication. Same day refill requests will be charged $35.
I am unable to provide care to patients while they are participating in the following treatment programs:
1. Hospitalization (either medical or psychiatric)
2. Residential care
3. Partial hospitalization
4. Intensive outpatient
Further, if I do not feel that outpatient care is safe, I will provide appropriate referrals for care and will accept you back once you are clinically stable for outpatient treatment.
I do not conduct one-time disability assessments, custody, fitness for duty, or other legal evaluations. I do not fill out paperwork for Emotional Support Animals.
I do not provide acute substance detoxification, medication assisted treatment with buprenorphine or methadone, court-mandated treatment, clozapine therapy, or medical marijuana prescriptions.
