EquiMind LLC Policies

High quality, comprehensive mental health care is in limited supply and recent events have only further strained the system. My policies align with my core value of mutual respect and allows me to continue to provide exceptional services to clients who need them.  

Intake Form and Consents for Treatment ​​

 

When you schedule our first meeting, you will receive an email with links to complete a confidential comprehensive intake packet. You will also receive and complete a consent for treatment, billing form, and any pertinent releases of information. This is required before your first appointment.

​Fees â€‹â€‹

 

I do not accept insurance and am considered an “out of network provider” for PPO plans. While I do not contract with insurance companies, I do assist my patients by providing the paperwork necessary to submit to their insurance carriers for reimbursement (a “superbill” with codes that reflect services provided). I am not able to negotiate or submit claims with insurance companies. Please consult your insurance provider in advance for information regarding limits to out of network care.  I cannot guarantee what portion of the bill your out of network insurance company will pay, and I do not contact the out of network insurance on your behalf. EquiMind LLC does not participate in Medicare or Medicaid and you cannot submit any bills from EquiMind to Medicare/Medicaid.

All clients must have an active credit card on file in order to receive treatment at EquiMind and payment is due in full at the time of service. You are responsible for fees from credit card companies, collection agencies or banks due to insufficient funds, payment disputes, or non-payment of fees. Please notify me if there is any change in your payment information or if any problems arise in your ability to make payments.

​​Initial Comprehensive Evaluation (60 minutes): $350

Follow-up Meetings (30 minutes-45 minutes): $250

Phone calls, forms, and other paperwork requested by you that require more than 5      minutes to complete outside of scheduled appointments will be billed at my prorated    rate.

Cancellation and No Show Policy

​

Notice of cancellation must be received at least 24 business hours in advance of your appointment. Business hours are considered weekdays from Monday through Friday and exclude all standard holidays. Late cancellation for our initial meeting may not be rescheduled, based on multiple factors. If you do not come in for our initial meeting without contacting me beforehand, you will not be able to reschedule and will need to find another mental health provider. 

 

Cancellations made prior to one business day before the appointment are not charged. If a cancellation occurs less than one business day before the appointment, you will incur a charge equal to half of the visit cost. For missed appointments, you will be charged the full cost of the visit. Arriving more than 15 minutes late is considered a missed appointment and will need to be rescheduled. These fees cannot be submitted for insurance reimbursement. Given potential unforeseen circumstances, for established clients the first follow-up late cancellation or missed appointment​ will be forgiven.

Your scheduled appointment time is reserved time in which you have my full attention. If you arrive late, we will finish at the scheduled time. Extending your appointment time would result in delays for other clients. Repeated missed appointments or late cancellations may be cause to discontinue treatment.

 

In the event that I need to reschedule an appointment, I will make every effort to provide you with adequate notice and will work to reschedule your appointment in a timely fashion.​​​

Medication Refills

​

Due to concerns of medication accuracy and concerns for your privacy, I do not respond to pharmacy-generated automated refill requests.  Before requesting medication refills, please contact the pharmacy directly to ensure that you do not have any additional refills or a new prescription available. 

 

As a result of increases in prior authorizations from insurance companies, advocating on your behalf takes longer and adds additional administrative work for providers. To account for this, please contact me for medication refills 5 days prior to needing the medication. I cannot guarantee your medication will be filled the same day.

Scope of Practice​

​

I am unable to provide care to patients while they are participating in the following treatment programs:

 

1.    Hospitalization (either medical or psychiatric)

2.   Residential care

3.   Partial hospitalization

4.   Intensive outpatient 

 

Further, if I do not feel that outpatient care is safe, I will provide appropriate referrals for care and will accept you back once you are clinically stable for outpatient treatment.

 

I do not conduct disability, custody, fitness for duty, or other legal evaluations. I do not fill out paperwork for Emotional Support Animals. 

 

I do not provide acute substance detoxification, medication assisted treatment with buprenorphine or methadone, court-mandated treatment, clozapine therapy, or medical marijuana prescriptions.

If there is a crisis and you are not able to immediately get in touch with me, please do NOT wait for me to return your message. Instead, act to keep yourself or your loved one safe. Call 988 or go to the nearest emergency room. You may also try the following crisis services:
