Fill out a HIPPA-Compliant Consultation Request Form after reviewing the EquiMind Policies page and I will contact you as soon as possible (typically within 1-2 business days).
I am in-network with United/Optum only. For all other insurance plans, I am considered an "out of network provider." This means payment is due at the time of service and most FSA/HSA spending plans are accepted as payment. I can provide you with documentation that you may submit to your insurance carrier for possible out-of-network reimbursement depending on your plan (typically most PPO plans).
I recommend you consult your insurance provider in advance regarding deductibles and coverage for out of network care.  Unfortunately, I am not able to negotiate with insurance companies. EquiMind LLC does not participate in Medicare and legally cannot see patient's with Medicaid insurance plans.
I am in-network with United/Optum. For patients with United/Optum plans, your co-pay and deductible depends on your specific plan. I recommend you contact your insurance carrier to clarify your behavioral health coverage.
If I am out-of-network with your insurance provider, my fees are listed below:
Initial Consultation (60 minutes): $350
Follow-up Appointments (30 minutes- 50 minutes): $175-$250, depending on services provided​​

**I can provide you with documentation you can submit to your insurance for partial reimbursement if your plan has out-of network benefits.
We will work together to determine how to best meet your needs while keeping treatment affordable for you. I do require patients to be seen at least every 3 months.
Medication management only appointments are 30 minutes and follow up frequency is variable based on a number of factors.
I also offer a limited number of patient spots for combined therapy and medication management that are typically seen weekly and appointments are 50 minutes.
When clinically indicated, I believe controlled substances can be a valuable component of a treatment plan. However, I generally do not prescribe controlled substances during our initial consultation period and certain clinical presentations may require outside testing or prior records to ensure appropriate treatment course. I do not prescribe medical marijuana.
