Fees and Reimbursement​​

​I am in network with most commercial UPMC and United/Optum plans. Self-pay options are also available. If I am out of network with your insurance, payment is due at time of service. I can provide you with documentation including what you paid along with medical billing codes that reflect services provided which you can submit to your insurance for reimbursement if you have out-of-network coverage. I am not able to negotiate with insurance companies. Please consult your insurance provider in advance for information regarding behavioral health coverage. 

All patients must have an active credit card on file to receive care at EquiMind LLC even if utlizing insurance to cover co-pays and other incurred fees not covered by insurance. Most FSA/HSA spending cards are also accepted as payment.

Self-pay rates:

​Initial Consultation: $350​

30-minute follow-up (Med Management Only): $175 ​

50-minute follow-up: $250​​

Other Fees:

Preparation of Medical Records: $25

Same Day Refill Request: $35

​

Phone calls, messaging, forms, reports, and other paperwork that require more than 5 minutes to complete outside of scheduled appointments will be billed at $350/hour.
